Responsible for outreach activities, initial intakes and program screening, referrals, file management, data entry, reporting and short-term case management services and assistance to homeless veterans and veteran families in order to secure or maintain permanent housing. Assist program participants in obtain VA and other public benefits. Assist participants in building life and fiduciary skills to create housing sustainability.
- Conduct extensive outreach in the community at large and with network providers to increase the utilization and outcomes
- Perform initial client intakes and program screening and assist clients in qualification for all programs utilizing a Housing First model
- Provides services to assist participants in developing needed skills of daily living and housing stabilization. Ensures that individual and/or household sessions focus on areas relevant to participants' goals for employment and homeless prevention and rapid re-housing including Family Development, Hope University coursework, Life Skills Coaching, resource coordination, advocacy, and crisis intervention
- Provides and documents referrals to any applicable resources such as the VA, local Veteran's Programs, Worksource, Washington State Department of Veteran Affairs, or other appropriate resources.
- Works with participants to obtain and become stable in permanent housing that is appropriate and adequate to participants needs. Provides liaison role between property owners/managers and program participants.
- Ensures that participants have access to available housing resources, and ensures that participants are able to pay rent, utilities and manage affairs necessary to remain stable in their housing. Helps participants build the skills to keep their living quarters clean and safe.
- Provides ongoing assessment and provision of service according to individual housing plan.
- Ensures participants meet criteria for HPRP as designed by the program and ensures timelines are met for rapidly re-housing clients.
- Works with participants to obtain relevant vocational training, basic or advanced education according to their level of functioning, needs and desires.
- Ensures billings and all data collection meet and follow agency policies and procedures, and the SSVF data collection guidelines.
- Maintains records in keeping with agency, county and state and federal guidelines.
- Works with other local agencies in providing services to shared clients
- Responsible for recruiting local housing for participation in the program and keeping the inventory of potential housing ahead of need
- Responsible for recruiting potential clients for the program at levels that satisfy all grant requirements and program strategies
- Maintain accurate case file documentation, client characteristics, and program utilization information for all program clients in CAP60 database and other required reporting systems
- Coordinate preparation of program reports, as required by funding source and the Program Manager.
- Coordinate preparation of program billings as required by the Program Manager and funding sources.
- Represent the agency in public setting.
- Assists the Program Manager in developing program structures and service delivery mechanisms to enhance, expand, and improve services for veterans and families
- Attends staff trainings, staff meetings, and other professional trainings as required
- Understand firmly each grant and the contract requirements.
- Responsible to understand coding (GL/Dept/Grant/Expense) for PO's and to properly code and allocate activity reporting.
- Responsible to know program budgets and available spends.
- Meet organization's documentation requirements for program adherence, finance, and file accuracy.
- Responsible for coordinating with service agency's monthly developing working relationships.
- Undergraduate degree in related field or equivalent experience
- Minimum of one-year successful experience working with individuals or families requiring assistance
- Experience working with individuals of diverse backgrounds and disabilities including physical, mental, substance abuse, etc.
- Ability to handle difficult situations and diverse cultural backgrounds in a discreet and professional manner
- Ability to respond appropriately in a crisis situation
- Ability to network effectively and build positive relationships internally and externally to the organization.
- Outstanding organizational, verbal and written communication skills and detail-oriented
- Proficient in Microsoft Office Suite and IT network usage
- Ability to perform daily duties to achieve expected outcomes and performance measures with minimal supervision
- Ability to travel to various sites daily if needed, vehicle or mileage provided
